Early Intervention Providers
Speech Therapist
Position Summary: An early intervention speech therapist provides specialized treatment to toddlers age birth-3. A speech therapist will work with a family to help their child communicate basic wants and needs, and enhance language skills. A KEIS speech therapist will conduct five area assessments to assess progress in all 5 domains of development. Essential Job functions for all EI providers: - Provide discipline-specific services for the frequency and duration listed on the child’s IFSP (individualized family service plan) - Collaborate with caregivers to ensure therapy is focused around family priorities and outcomes - Attend IFSP meetings (6-month, annual, and requested reviews) via a telehealth platform or in-person - Conduct annual five area assessments - Complete service notes in a timely manner (10 days) in the TOTS portal - Document your services to be processed for payroll (weekly) Requirements: - Master’s degrees in speech-language pathology - State licensure issued from the Kentucky Board of Speech-Language Pathology
Physical Therapist
Position Summary: An early intervention Physical therapist (PT) addresses physical developmental delays such as delays in gross motor skills, fine motor skills, and sensory processing in a toddler age birth-3. Physical therapy is designed to the individual needs of the child and family but generally involves exercises to enhance strength /coordination, adaptive equipment recommendations, parent coaching/guidance, and therapeutic activities. Essential Job functions for all EI providers: - Provide discipline-specific services for the frequency and duration listed on the child’s IFSP (individualized family service plan) - Collaborate with caregivers to ensure therapy is focused around family priorities and outcomes - Attend IFSP meetings (6-month, annual, and requested reviews) via a telehealth platform or in-person - Conduct annual five area assessments - Complete service notes in a timely manner (10 days) in the TOTS portal - Document your services to be processed for payroll (weekly) Requirements: - Doctor of Physical Therapy (DPT) degree - State licensure
Occupational Therapist
Position Summary: An early intervention occupational therapist (OT) works with toddlers age birth-3. The OT promotes function and engagement in a child’s participation in activities of daily living (ADLs), such as mealtimes, play, sleep, and social participation. Essential Job functions for all EI providers: - Provide discipline-specific services for the frequency and duration listed on the child’s IFSP (individualized family service plan) - Collaborate with caregivers to ensure therapy is focused around family priorities and outcomes - Attend IFSP meetings (6-month, annual, and requested reviews) via a telehealth platform or in-person - Conduct annual five area assessments - Complete service notes in a timely manner (10 days) in the TOTS portal - Document your services to be processed for payroll (weekly) Requirements: - Master’s degree in Occupational therapy - State licensure
Developmental Interventionist
Position Summary: A developmental interventionist (DI) addresses developmental delays in toddlers age birth-3. A DI works collaboratively with a child’s family to identify priorities, concerns, and needs regarding their child’s overall development. A DI has knowledge and expertise in all 5 domains of development: motor, cognitive, communication, self help, and social emotional. Essential Job functions for all EI providers: - Provide discipline-specific services for the frequency and duration listed on the child’s IFSP (individualized family service plan) - Collaborate with caregivers to ensure therapy is focused around family priorities and outcomes - Attend IFSP meetings (6-month, annual, and requested reviews) via a telehealth platform or in-person - Conduct annual five area assessments - Complete service notes in a timely manner (10 days) in the TOTS portal - Document your services to be processed for payroll (weekly) Requirements: - Bachelor’s degree and an interdisciplinary early childhood education (IECE)
Adult Foster Care Roles
Behavior Support Specialist
Position Summary: The Behavior Specialist provides community-based therapy support services. Adults and children with developmental disabilities are the primary population supported by the Behavior Specialist. The Behavior Specialist provides a functional behavior assessment along with the development of a behavior support plan and ensures the implementation and monitoring of that plan across settings. As a member of the person-centered team, the Behavior Specialist will utilize the most current evidence-based strategies available in applied behavior analysis. Essential Job Functions: - Complete functional assessment within 60 days of service initiation, which includes: - An analysis of the potential communicative intent of the behavior; - The history of the reinforcement of the behavior; - The critical variables that preceded the behavior; - Effects of different situations, or environmental variables, on the behavior; and - A hypothesis regarding the motivation, purpose, and factors that maintain the behavior. - Develop, implement, and monitor Behavior Support Plan using positive behavior supports, including: - Definition of techniques and procedures used; - Be designed to equip the participant to communicate his or her needs and to participate in age-appropriate activities; - Include the hierarchy of behavior interventions ranging from the least to most restrictive; - Reflect the use of positive approaches; and - Prohibit the use of restraints, seclusion, corporal punishment, verbal abuse, and any procedure that denies private communication, adequate sleep, shelter, bedding, food, drink, or use of a bathroom facility; and - Include the provision of the training to other MPW providers concerning implementation of the BSP. - Provide services in a timely manner, within 15 days of service authorization. - Complete service requirements as outlined in SCL, MPW regulations. - Complete quarterly reports which include both behavior data and verification of time spent with participant. - Complete notes for each contact made with participant, including the beginning and ending time, date and location of service, description of service provided, including progress on meeting outcomes and goals identified in BSP, along with name, title, date and signature of person writing note. - Provide graphical representations of behavior data in monthly, quarterly, and annual formats - Attend POC meetings, quarterly staff meetings, and psychiatric appointments when needed. - Maintain constant communication with Case Manager and other members of the person-centered team - Be available or have established an available back up, by phone, 24/7. - Submit Critical Incident Reports to DCBS/DDID and complete necessary follow-up documentation and follow all procedures as required by DDID - Follow all necessary documentation requirements as directed - Performs other duties as needed Requirements: - Master’s degree or higher from an accredited institution with formal graduate coursework in behavioral science - Board certified, or BCBA eligible - Continuing education, if applicable - Minimum one year experience in behavioral programming for individuals with intellectual and developmental disabilities - Two years of field experience in an organization which serves individuals with intellectual or developmental disabilities
Person Centered Coach
Position Summary: The Person Centered Coach (PCC) provides community-based support services. Adults with developmental disabilities are the primary population by the Person Centered Coach. The Person Centered Coach, in conjunction with the Behavior Specialist and Case Manager, ensure the proper implementation of person centered support plans. Essential Job Functions: - Ensure that all behavior support plans are implemented, as written - Implement, train and monitor behavior support plan using positive behavior supports - Assists a participant and the participant’s person-centered team in implementing and monitoring the effectiveness of the participants person-centered service plan - Models person-centered thinking - Is responsible for training a participant, family, guardian, natural and unpaid supports, and other members of the person-centered team when barriers challenge the success of the participant in achieving his or her goals - Provide services in a timely manner, within 15 days of service authorization - Complete service requirements as outlined in SCL, MPW regulations - Complete quarterly reports which include both behavior data and verification of time spent with participant - Attend POC meetings, quarterly staff meetings, and psychiatric appointments when needed - Maintain constant communication with Behavior Specialist, Case Manager and other members of the person-centered team - Be available or have established an available back up, by phone, 24/7 - Submit Critical Incident Reports to DSBS/DDID and complete necessary follow-up documentation and follow all procedures as required by DDID - Follow as necessary documentation requirements as directed - Performs other duties as needed Requirements: - A high school diploma or GED; and either two (2) years of experience in the field of intellectual or developmental disabilities; or completed twelve (12) hours of college coursework in a human services field - Continuing education, if applicable - Minimum two years of experience in the field of developmental disabilities - 12 hours of college credits in a human service field if 2 years of experience is not met
Community Access Specialist
Position Summary: The Community Access Specialist will provide support and training to a waiver participant that enables that participant to develop a network of natural supports to achieve a clearly defined and valued social role within the participant’s community. Essential Job Functions: - Services shall be designed to support a participant to participate in meaningful routines, events, and activities through various community organizations. - Shall be designed to empower a participant in developing natural supports - May be participant directed and provided by a family member, guardian, or legally responsible individual of the participant in accordance to regulation - Shall stress training that empowers a participant in acquiring, practicing, utilizing and improving skills related to: Connecting with others, independent functioning, self-advocacy, socialization, community participation, personal responsibility, financial responsibility, other skills related to optimal well- being as defined by the participants person-centered plan. - Shall assist the participant to increase ability to develop natural supports and access community resources including education, recreational, religious, civic or volunteer opportunities with an outcome of less reliance on formal supports and greater reliance on natural or unpaid supports as established in the person-centered service plan. - Shall assist the participant in development of personal social networks, membership opportunities, friendships, and relationships for the participant as established in the participant’s person-centered service plan. - Shall provide services outside the participants home or residential setting and occur during the day, evening or on weekends. - Shall not duplicate residential support or day training services or authorized therapies - Shall provide 1:1 services, staff to participant ration or no more than two participants according to the person- centered service plan, if the participant invites a friend - Shall provide services in an integrated community setting - Shall provide an impact service and the participants plan shall define steps to decrease the provision of the service as the participant becomes more independent in accessing and becoming part of the community - Be available or set up back up at minimal by phone 24/7. - Complete a service note that includes full description of service rendered, evidence of training of service to support outcomes designated in the person- centered service plan, date of service, location of the service, beginning and ending time of service, signature and title of the individual providing the service, date entry was made, signed with the title and credentials of the person providing the service. - Any other responsibility assigned by supervisor. Requirements: - Has previously qualified or been credentialed by the department to provide community access. - 1-year of experience in the field of IDD and completed a department approved training program within one year of application while providing community access services under the direct supervision of a community access specialist Meets the personnel and training requirements in section 3 of 10 of the regulation 907 KAR 12:010. - High School Diploma - Continuing education if applicable
Community Living Supports
Position Summary: The CLS position provides a service to facilitate independence and promote integration into the community for a participant residing in his or her home or in his or her family’s home served under the Michelle P. (MPW) Medicaid waivers. Essential Job Functions: - Provide supports and assistance related to chosen outcomes on client’s PSCP - Ensure supports are in compliance with Medicaid regulations - Collaborate effectively with each participant’s case manager and other support providers/agencies to ensure that services are person centered - Advocate for and be a resource to participants and their families. Allow self-determination and input from families to guide services - Practice safety and report all workplace accidents according to KPI Therapy policies - Ensure incident reports and follow up incident reports are filed and sent to case managers in a timely manner for all incidents concerning participants - Perform all other related duties, as required by the SCL and Michelle P. Medicaid waiver programs - Maintain cordial working relationships with outside agencies providing waiver services/case management - Ensure that billing issues are addressed and help the accounting department troubleshoot any Medicaid payment errors Requirements: - 18 years of age - Pass background check - Pass drug screen
