KPI's Adult Foster Care

Adult foster care (AFC) is a residential option for adults who need support with daily living. In an AFC setting, individuals live in a provider’s home where they receive 24-hour supervision, personal care, and assistance, along with room, board, and laundry services. AFC emphasizes a family-centered environment and integrates everyday community supports, such as interactions with family and friends and participation in job or training activities. The program is rooted in the belief that people are most fulfilled when they can live active and meaningful lives within their own communities.

Who is AFC for?

  • People living in AFC homes are men and women, 18 years of age and older. Who, because of mental or physical disability, cannot manage on their own anymore.

  • This living arrangement is intended for single persons or partners, with no more than four such persons in a single-family home.

  • The care may be provided by people over the age of 21 who meet the qualifications and criteria established for AFC providers.

  • People living in AFC homes need support to remain within or to return to the community from an institutional setting.

  • All individuals’ needs and abilities vary, but remaining in the community is a common goal.

How AFC Works?

Case managers, hospitals, and other entities will contact our team when there is a potential need for a residential placement. We will then check the availability and willingness of foster care homes to accept a new resident. A  pre-placement visit is scheduled to serve two purposes: first, to allow the prospective resident to see the house and surrounding community, and second, to enable the resident, provider, and any potential roommates to meet and determine if the living arrangement is a good fit. If everyone agrees that the arrangement is suitable, we will proceed with the placement plans.

In adult foster care (AFC), the expectations are as follows:

  • For Residents:

    • Adapt to new routines and environments.

    • Actively participate in daily activities and community life.

    • Communicate needs, preferences, and concerns openly.

  • For Providers:

    • Offer 24-hour supervision, personal care, and necessary assistance.

    • Foster a respectful and inclusive environment.

    • Maintain open communication with residents, their families, and case managers.

Both residents and providers should collaborate to ensure a positive and supportive living experience within the community.

What is Michelle P. Waiver?

Michelle P. Waiver (MPW) is part of Kentucky's 1915 Home and Community-Based Services (HCBS) Medicaid waiver program. It provides support to individuals with intellectual or developmental disabilities, helping them live independently in their communities.

 

The waiver was established after a 2002 lawsuit by Michelle P., who highlighted the state's waitlist of over 3,000 people for other waivers. In 2008, Kentucky agreed to create the MPW to better serve those in need of these services.

MPW Services

The Michelle P. Waiver (MPW) offers a range of services tailored to support each participant's goals and preferences. These services include:

  • Behavioral Supports

  • Day Training

  • Environmental and Minor Home Adaptation

  • Personal Care

  • Respite

Participants in the MPW program have the flexibility to choose how their non-medical, non-residential services are delivered. They can opt for a traditional provider agency, hire their own employees, or use a combination of both through Participant Directed Services (PDS). For more details, you can visit the DMS PDS page.

Eligibility 

You may qualify for MPW services if you meet the following criteria:

  1. Disability: You have an intellectual or developmental disability.

  2. Risk of Institutionalization: Without waiver services, you would require admission to an intermediate care facility for individuals with intellectual disabilities (ICF-IID) or a nursing facility.

  3. Need for Support: You require a protected environment to learn daily living skills, gain educational experiences, and develop environmental awareness.

  4. Financial Qualifications: You meet the Medicaid financial criteria, which includes specific qualifications for the MPW program. Refer to the Medicaid Waiver Services Fact Sheet for details.
